Community Health & Wellness Manager

The Community Health and Wellness Manager is a key member of the West Philly Promise Neighborhood team. The Manager will be responsible for overseeing all activities related to the health and wellness of children and their families. The Manager will work with partner agencies to design, implement, and monitor programs that foster lifelong healthy behavior. Additionally, the Manager will oversee the new Promise Neighborhood Navigator program which will connect parents and caregivers to resources and services.

Qualifications:

Bachelor’s degree or Higher in Social Work or related field including but not limited to sociology, psychology, counseling, education, or public health administration preferred.

Essential Functions:

Community Navigator Program
Co-develop the Promise Neighborhood Navigator model to ensure families have access to programs and services.
Manage operations and service delivery to meet the needs of program participants.
Supervise three Navigators.
Support Navigators in providing brief, time-limited information and referral services with the goal of increasing access to resources within the community.
Provide training and support to Navigators on best practices in participant engagement, client centered practice and trauma-informed care.

Partnerships and Collaborative Relationships:
Develop partnerships with external organizations to enhance service coordination and availability of resources for families within the West Philly Promise Neighborhood.

Advise and guide the grant implementation team on strategy related to health, wellness, safety and community capacity.
Participates in regular meetings with internal staff to ensure priorities are clear, coordination is strong and communications are open. Assist project leadership in the development of policies and procedures related to grant specific activities.

Demonstrated ability to develop, implement and follow project timelines to ensure completion of project deliverables on or ahead of schedule. Ability to identify and prioritize tasks based on project and organizational needs and manage sometimes-competing priorities.

Experience working with diverse populations, low-income individuals, and in a variety of community settings.
Excellent problem solving, conflict resolution, time management and professional communication (written and oral skills).
