Case Manager Elect

MCC seeks a Case Manager for the Education Leading to Employment and Career Training (ELECT) program to be responsible for providing culturally sensitive case management and group education services to pregnant and parenting students enrolled in school participating in the ELECT programs in Montgomery and Bucks counties.
The Case Manager will maintain relationships with identified clients and school personnel. Provide support to the participants through quarterly home visits, one on one sessions and group facilitation. In addition, the Case Manager will also attend community meetings and trainings, keep accurate written records, and will use the organization database.
 
Qualified candidates will be motivated individuals, knowledgeable about community resources, with good computer skills. Ability to work well with people, good organizational skills. Valid driver’s license and clean driving record. Minimum of Bachelor’s Degree required.
