YSRP seeks a Reentry Coordinator to provide integrated and holistic case management support for YSRP’s youth client-partners. The Reentry Coordinator will work in partnership with youth who have faced charges in the adult criminal justice system in Philadelphia, Montgomery, and Delaware Counties as they prepare to and then return to the community from detention or incarceration in adult and/or juvenile carceral settings. The Reentry Coordinator will employ a team-based approach to collaborating with YSRP colleagues who provide mitigation, reentry, and legal support, as well as with the young person’s defense counsel, to support the accomplishment of the youth client-partner’s self-determined goals. Supporting reentry goals may include, but is not limited to: facilitating connections to community-based employment, education, housing, and health-care resources. The Reentry Coordinator is a full-time, exempt, staff position that will report to YSRP’s Director of Advocacy. The Reentry Coordinator’s role is critical in effectuating YSRP’s comprehensive case advocacy model, which seeks to lessen the duration and impact of adult criminal sentences on children and families in and around Philadelphia.

The Reentry Coordinator will be responsible for the following core duties:

Comprehensive & Holistic Case Management

  • As a member of the legal defense team (lawyer(s) and mitigation specialist), create detailed, individualized, and comprehensive reentry plan; ensure it is part of the presentation made to the prosecutor, judge and/or the parole board.
  • In collaboration with mitigation specialist, support and advocate for young person in court.
  • Provide case management services to youth to ensure healthy connections to existing community resources in areas including housing, education, employment, and healthcare. Examples of support range from referrals and transportation for a client-partner to a job interview, assisting with housing and employment plans, or connecting them with other services that allow them to meet their self-determined goals.
  • Track and input case management activities in an online case management system, including data and metrics contemplated by YSRP’s strategic plan.
  • Conduct regular in-person and virtual visits with youth client-partners in carceral settings, including juvenile placement facilities, detention centers, adult jails and prisons across Pennsylvania.
  • Once the youth client-partner returns home, conduct regular in-person visits with them at home, school, place of employment, and in the community.
  • Conduct in-person visits with youth client-partner family members and members of the youth client-partner’s support system, including, but not limited to: teachers, coaches, social workers, program administrators, healthcare providers, and faith community leaders.
  • Liaise regularly with organizational partners and system actors, including probation and parole officers as needed.
  • In collaboration with the client-partners, prepare, track and maintain Mutual Responsibility Agreements, which provide incentive-based payments to YSRP client-partners for the achievement of their self-determined goals.

Community Partnerships

  • In collaboration with YSRP Program Coordinator and Senior Reentry Coordinator, develop and maintain relationships with community-based service providers and partner organizations.
  • Input and update community partner information in internal databases and other information management systems.
  • Interact regularly with community partners, including but not limited to: attending events, speaking on behalf of YSRP, responding to inquiries and requests for support, and liaising between community partners and other YSRP staff.

Qualifications:

  • At least 2-5 years of combined experience in case management, reentry advocacy and/or program administration
  • Experience working directly with youth ages 14-22 (approx.)
  • Experience working in and/or alongside youth-led movements
  • Familiarity with the day-to-day functioning of Pennsylvania’s criminal legal system a plus
  • Strong writing skills
  • Experience liaising with community-based organizations
  • Experience with Google Workspace a plus
  • Comfortable working remotely
  • Willingness to have flexible work schedule including some evenings, travel as needed, and responding to communication from youth client-partners and family members

Compensation:

The starting salary for the Reentry Coordinator is $65,000. This is a full-time, exempt position. YSRP offers a comprehensive benefits package, including 100% employer-paid health and dental insurance, retirement benefits, and generous paid time off.
